Patch series "Add support for fast mremap".
This series speeds up the mremap(2) syscall by copying page tables at
the PMD level even for non-THP systems. There is concern that the extra
'address' argument that mremap passes to pte_alloc may do something
subtle architecture related in the future that may make the scheme not
work. Also we find that there is no point in passing the 'address' to
pte_alloc since its unused. This patch therefore removes this argument
tree-wide resulting in a nice negative diff as well. Also ensuring
along the way that the enabled architectures do not do anything funky
with the 'address' argument that goes unnoticed by the optimization.
Build and boot tested on x86-64. Build tested on arm64. The config
enablement patch for arm64 will be posted in the future after more
testing.
The changes were obtained by applying the following Coccinelle script.
(thanks Julia for answering all Coccinelle questions!).
Following fix ups were done manually:
* Removal of address argument from pte_fragment_alloc
* Removal of pte_alloc_one_fast definitions from m68k and microblaze.

The 603 doesn't have a HASH table, TLB misses are handled by
software. It is then possible to generate page fault when
_PAGE_EXEC is not set like in nohash/32.
There is one "reserved" PTE bit available, this patch uses
it for _PAGE_EXEC.
In order to support it, set_pte_filter() and
set_access_flags_filter() are made common, and the handling
is made dependent on MMU_FTR_HPTE_TABLE

As done for book3s/64, add necessary flags/defines in
book3s/32/pgtable.h and do not include pte-common.h
It allows in the meantime to remove all related hash
definitions from pte-common.h and to also remove
_PAGE_EXEC default as _PAGE_EXEC is defined on all
platforms except book3s/32.

In order to avoid using generic _PAGE_XXX flags in powerpc
core functions, define helpers for all needed flags:
- pte_mkuser() and pte_mkprivileged() to set/unset and/or
unset/set _PAGE_USER and/or _PAGE_PRIVILEGED
- pte_hashpte() to check if _PAGE_HASHPTE is set.
- pte_ci() check if cache is inhibited (already existing on book3s/64)
- pte_exprotect() to protect against execution
- pte_exec() and pte_mkexec() to query and set page execution
- pte_mkpte() to set _PAGE_PTE flag.
- pte_hw_valid() to check _PAGE_PRESENT since pte_present does
something different on book3s/64.
On book3s/32 there is no exec protection, so pte_mkexec() and
pte_exprotect() are nops and pte_exec() returns always true.

Commit 667416f38554 ("powerpc/mm: Fix kernel crash on page table free")
added a call for pgtable_page_dtor in the rcu page table free routine. We missed
the fact that for 32 bit platforms we did call the 'dtor' early. Drop the extra
call for pgtable_page_dtor. We remove the call from __pte_free_tlb so that we
do the page table free and 'dtor' call together. This should help when we
switch these platforms to pte fragments.

With 4k page size for hugetlb we allocate hugepage directories from its on slab
cache. With patch 0c4d26802 ("powerpc/book3s64/mm: Simplify the rcu callback for page table free")
we missed to free these allocated hugepd tables.
Update pgtable_free to handle hugetlb hugepd directory table.

When relaxing access (read -> read_write update), pte needs to be marked invalid
to handle a nest MMU bug. We also need to do a tlb flush after the pte is
marked invalid before updating the pte with new access bits.
We also move tlb flush to platform specific __ptep_set_access_flags. This will
help us to gerid of unnecessary tlb flush on BOOK3S 64 later. We don't do that
in this patch. This also helps in avoiding multiple tlbies with coprocessor
attached.

To support memory keys, we moved the hash pte slot information to the
second half of the page table. This was ok with PTE entries at level
4 (PTE page) and level 3 (PMD). We already allocate larger page table
pages at those levels to accomodate extra details. For level 4 we
already have the extra space which was used to track 4k hash page
table entry details and at level 3 the extra space was allocated to
track the THP details.
With hugetlbfs PTE, we used this extra space at the PMD level to store
the slot details. But we also support hugetlbfs PTE at PUD level for
16GB pages and PUD level page didn't allocate extra space. This
resulted in memory corruption.
Fix this by allocating extra space at PUD level when HUGETLB is
enabled.

pte_access_premitted get called in get_user_pages_fast path. If we
have marked the pte PROT_NONE, we should not allow a read access on
the address. With the current implementation we are not checking the
READ and only check for WRITE. This is needed on archs like ppc64 that
implement PROT_NONE using _PAGE_USER access instead of _PAGE_PRESENT.
Also add pte_user check just to make sure we are not accessing kernel
mapping.
Even though there is code duplication, keeping the low level pte
accessors different for different platforms helps in code readability.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

gup_hugepte() checks if pages are present and readable, and
when 'write' is set, also checks if the pages are writable.
Initially this was done by checking if _PAGE_PRESENT and
_PAGE_READ were set. In addition, _PAGE_WRITE was verified for write
accesses.
The problem is that we have to handle the three following cases:
1/ The target defines __PAGE_READ and __PAGE_WRITE
2/ The target defines __PAGE_RW
3/ The target defines __PAGE_RO
In case 1/, this is obvious
In case 2/, __PAGE_READ is defined as 0 and __PAGE_WRITE as __PAGE_RW
so it works as well.
But in case 3, __PAGE_RW is defined as 0, which means __PAGE_WRITE is 0
and then the test returns true (page writable) in all cases.
A first correction was attempted in commit 6b8cb66a6a7cc ("powerpc: Fix
usage of _PAGE_RO in hugepage"), but that fix is wrong:
instead of checking that the page is writable when write is requested,
it checks that the page is NOT writable when write is NOT requested.
This patch adds a new pte_read() helper to check whether a page is
readable or not. This avoids handling all possible cases in
gup_hugepte().
Then gup_hugepte() is modified to use pte_present(), pte_read()
and pte_write() instead of the raw flags.

Introduce a helper pgtable_gfp_flags() which
just returns the current gfp flags and adds
__GFP_ACCOUNT to account for page table allocation.
The generic helper is added to include/asm/pgalloc.h
and has two variants - WARNING ugly bits ahead
1. If the header is included from a module, no check
for mm == &init_mm is done, since init_mm is not
exported
2. For kernel includes, the check is done and required
see (3e79ec7 arch: x86: charge page tables to kmemcg)
The fundamental assumption is that no module should be
doing pgd/pud/pmd and pte alloc's on behalf of init_mm
directly.
NOTE: This adds an overhead to pmd/pud/pgd allocations
similar to x86. The other alternative was to implement
pmd_alloc_kernel/pud_alloc_kernel and pgd_alloc_kernel
with their offset variants.
For 4k page size, pte_alloc_one no longer calls
pte_alloc_one_kernel.

If an architecture uses 4level-fixup.h we don't need to do anything as
it includes 5level-fixup.h.
If an architecture uses pgtable-nop*d.h, define __ARCH_USE_5LEVEL_HACK
before inclusion of the header. It makes asm-generic code to use
5level-fixup.h.
If an architecture has 4-level paging or folds levels on its own,
include 5level-fixup.h directly.

Today powerpc64 uses a set of pgtable_caches while powerpc32 uses
standard pages when using 4k pages and a single pgtable_cache
if using other size pages.
In preparation of implementing huge pages on the 8xx, this patch
replaces the specific powerpc32 handling by the 64 bits approach.
This is done by:
* moving 64 bits pgtable_cache_add() and pgtable_cache_init()
in a new file called init-common.c
* modifying pgtable_cache_init() to also handle the case
without PMD
* removing the 32 bits version of pgtable_cache_add() and
pgtable_cache_init()
* copying related header contents from 64 bits into both the
book3s/32 and nohash/32 header files
On the 8xx, the following cache sizes will be used:
* 4k pages mode:
- PGT_CACHE(10) for PGD
- PGT_CACHE(3) for 512k hugepage tables
* 16k pages mode:
- PGT_CACHE(6) for PGD
- PGT_CACHE(7) for 512k hugepage tables
- PGT_CACHE(3) for 8M hugepage tables

When we are updating a pte, we just need to flush the tlb mapping
that pte. Right now we do a full mm flush because we don't track the page
size. Now that we have page size details in pte use that to do the
optimized flush

Even though a tlb_flush() does a flush with invalidate all cache,
we can end up doing an RCU page table free before calling tlb_flush().
That means we can have page walk cache entries even after we free the
page table pages. This can result in us doing wrong page table walk.
Avoid this by doing pwc flush on every page table free. We can't batch
the pwc flush, because the rcu call back function where we free the
page table pages doesn't have information of the mmu gather. Thus we
have to do a pwc on every page table page freed.
Note: I also removed the dummy tlb_flush_pgtable call functions for
hash 32.

This patch start to make a book3s variant for pgalloc headers. We have
multiple book3s specific changes such as:
* 4 level page table
* store physical address in higher level table
* use pte_t * for pgtable_t
Having a book3s64 specific variant helps to keep code simpler and remove
lots of #ifdef around code.

pmd_hugepage_update() is inside #ifdef CONFIG_TRANSPARENT_HUGEPAGE. THP
can only be enabled if PPC_BOOK3S_64=y && PPC_64K_PAGES=y, aka. hash64.
On hash64 we always define PTE_ATOMIC_UPDATES to 1, meaning the #ifdef
in pmd_hugepage_update() is unnecessary, so drop it.
That is also the only use of PTE_ATOMIC_UPDATES in any of the hash code,
meaning we no longer need to #define it at all in the hash headers.
Note it's still #defined and used in the nohash code.
