With the decline of ISDN, this seems to have become almost completely
obsolete, and even in the past years before that, almost all remaining
users appear to have used mISDN instead.
Birger Harzenetter noted that he is still using i4l/hisax to take
advantage of the 'divert' driver for call diversion, but otherwise uses
mISDN on the same hardware. This is a rare edge case as far as I
can tell, but we are still breaking an actively used work flow
(see https://xkcd.com/1172/).
We debated moving i4l/hisax to staging as an intermediate step, but as
he is not likely to change the setup, and that would just delay breaking
this use case. The alternatives here are to stay on stable kernels
< 5.2, to create an external driver repository for isdn4linux, or to
add divert functionality to mISDN.

In my attempt to hide the warnings because I thought they didn't serve
any purpose[1], Masahiro Yamada pointed out that {Read,Write}_hfc in
hci_pci.c should be using a standard register access method; otherwise,
the compiler will just remove the if statements.
For hfc_pci, use the versions of {Read,Write}_hfc found in
drivers/isdn/hardware/mISDN/hfc_pCI.h while converting pci_io to be
'void __iomem *' (and clean up ioremap) then remove the empty if
statements.
For hfc_sx, {Read,Write}_hfc are already use a proper register accessor
(inb, outb) so just remove the unnecessary if statements.

isdn source code uses a not-current coding style.
Update the coding style used on a per-line basis
so that git diff -w shows only elided blank lines
at EOF.
Done with emacs and some scripts and some typing.
Built x86 allyesconfig.
No detected change in objdump -d or size.

- make needlessly global code static
- remove the compiled but unused st5481_hdlc.{c,h}
- kill enternow.h
- enternow_pci.c: kill InByte/OutByte/BYTE
- isdnl2.c: kill FreeSkb
- remove or #if 0 the following unused functions:
- config.c: IsdnCardState
- ipacx.c: ipacx_new_ph
- ipacx.c: dch_bh
- ipacx.c: setup_ipacx
- isdnl2.c: IsRR

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
