While setting up the virtual memory mapped IO to the OCMB chips we
make some assumptions that the OCMB MMIO spaces will be contiguous.
The p9a_omi_setup_bars HWP uses OMI_INBAND_BAR_BASE_ADDR_OFFSET to
set the scom registers that determine the physical offset mapped
to the IO. When setting up the Virtual addresses hostboot uses to
represent the physical mmio address, we must validate that the attribute
matches with what we calculated. While doing this we found that the
virtual address attribute was being calculated incorrectly. It was
not localizing the OCMB position relative to the MC which is required
when calculating the offset into the MC bar.

-A testcase was executed which garded out all the MBA's
in the system, the expectation was that hostboot would
attempt to configure enough hardware in the system to
successfully ipl. However instead of IPLing multiple
checkstops and various error logs were observed.
-The testcase to gard all the mba's and then ipl the
system resulted in there being no useable memory available
for the master proc (proc0). In this case there is code
which will swap the memory map around such that the sytem
can still IPL. One of the requirements to enable the
ability to swap the memory map around is to use the
effective chip and group ids for calculating the memory
ranges.
-When setting the inband scom address for the DMI targets
the existing calulation is using the default fabric chip
group ids, this commit will change the calulation to use
the "effective" chip and group ids when calculating the
inband scom mmio range.

The AbaPtr makes assumptions that we have pointer values that
are only 32-bits long. In our runtime (HBRT) environment that
assumption is not true so we need to make sure none of the code
that runs there is trying to use the AbaPtr code.

A channel failure can cause an inband scom access
to fail. This change will deconfigure the membuf
that failed to allow the IPL to continue without
that resource.

This commit updates IBSCOM to have error path support.
It also updates the good-path test cases since there is
limited good path support in simics. Full enablement
will be done later.

When I made my initial attempt at the IBSCOM Device Driver,
the proc_cen_setup_bars procedure had not yet been imported into
Hostboot code, so I was doing it manually in the Device Driver.
The procedure is now in Hostbood code, so my workaround is not
needed.
