Remove obsolete entries from fixincludes.
fixincludes/:
* inclhack.def (isc_fmod, isc_omits_with_stdc)
(nested_sys_limits, sco_math): Remove.
* tests/base/math.h (ISC_FMOD_CHECK, SCO_MATH_CHECK): Remove.
* tests/base/stdio.h (ISC_OMITS_WITH_STDC_CHECK): Remove.
* tests/base/sys/limits.h: Remove.
* fixincl.x: Regenerate.
