Verifier: Add operand checks for MDLexicalBlock
Add operand checks for `MDLexicalBlock` and `MDLexicalBlockFile`. Like
`MDLocalVariable` and `MDLocation`, these nodes always require a scope.
There was no test bitrot to fix here (just updated the serialization
tests in test/Assembler/mdlexicalblock.ll).
