AMDGPU: Rename isAmdCodeObjectV2 -> isAmdHsaOrMesa
The isAmdCodeObjectV2 is a misleading name which actually checks whether the os
is amdhsa or mesa.
Also add a test to make sure we do not generate old kernel header for code
object v3.
