Restructuring of token annotation for formatting.
This combines several changes:
* Calculation token type (e.g. for * and &) in the AnnotatingParser.
* Calculate the scope binding strength in the AnnotatingParser.
* Let <> and [] scopes bind stronger than () and {} scopes.
* Add minimal debugging output.
