[clang-tools-extra] Format sources with clang-format. NFC.

Summary:
Ran clang-format on all .c/.cpp/.h files in clang-tools-extra.
Excluded the test, unittests, clang-reorder-fields, include-fixer, modularize and pptrace directories.
