These board files only use gpio_keys not gpio in general.  This include is
just surplus, delete it.

The Kconfig currently controlling compilation of this code is:

  arch/x86/Kconfig:config NET5501
  arch/x86/Kconfig:       bool "Soekris Engineering net5501 System Support (LEDS, GPIO, etc)"

...meaning that it currently is not being built as a module by anyone.

Lets remove the couple traces of modularity, so that when reading
the driver there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

We also delete the MODULE_LICENSE tag etc. since all that information
is already contained at the top of the file in the comments.

The Kconfig currently controlling compilation of this code is:

  arch/x86/Kconfig:config ALIX
  arch/x86/Kconfig:       bool "PCEngines ALIX System Support (LED setup)"

...meaning that it currently is not being built as a module by anyone.

Lets remove the modular code that is essentially orphaned, so that
when reading the driver there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

We replace module.h with moduleparam.h since the file does declare
some module parameters, and leaving them as such is currently the
easiest way to remain compatible with existing boot arg use cases.

We also delete the MODULE_LICENSE tag etc. since all that information
is already contained at the top of the file in the comments.

It seems that there was an error with the active_low = 1 for the
LED, since it should be set to 0 (meaning that active is high,
since 0 is false, hence the confusion.

The wiki article about it confuses it, since it contradicts itself,
regarding what turns on the LED.

I have tested 3.4-rc2 on my net5501 with this patch, and it makes the LED
behave correctly, where "none" turns it off, and "default-on" turns it on,
when echoed onto the trigger "file" in /sys/class/leds.
