rcu: Renamings to increase RCU clarity
Make RCU-sched, RCU-bh, and RCU-preempt be underlying
implementations, with "RCU" defined in terms of one of the
three. Update the outdated rcu_qsctr_inc() names, as these
functions no longer increment anything.
