Resolve warnings when compiling with gcc 4.8. Compiled with GCC 7.3,
no more compile errors/warnings; build ends with caught exception from
linker. This commit compiles with GCC 8.2, no more error/warnings; except
for a linking warning.

Generally adds support to declare variables as thread_local
- Add support in HBRT start assembly to skip adjusting TLS relocations
- Add support in linker to generate tagged TLS entries
- Update linker to process TLS relocations correctly
- Update TLS code to ignore top half of module ID
- Update module images to hold a "module ID"
- Update custome linker to update module ID during binary link

There is an issue where if there are crit_asserts in the mutex
lock/unlock functions that the code will assert in a testcase
intermittently. This commit will remove those crit_asserts to alleviate
the issue until it can be investigated further.

We are still seeing some very intermittent errors in the slave
core wakeup path. It still seems like we may have a timing issue.
Until we figure out exactly what is going on, I am adding a retry
mechanism that should get the core to report in correctly. The
retry is done by issuing an additional doorbell message to the
core that didn't report in.

This commit introduces the changes to read out the SMF secure
memory amount value from NVRAM and to distribute the secure
memory amount based on the value read. strtou64 was copied
from runtime code to convert the value read from NVRAM (as a
string) to uint64_t.

There is an intermittent CI fail where test case modules will time out
due to an assert in sync.C. This will prevent the assert from occuring
if too many calls to mutex unlock are made.

This commit implements a compile time feature (set MALLOC_FENCING to
y in the src/lib/HBconfig file) which puts check bytes around each malloc.
Whenever an allocation is freed, if the check bytes have changed, the memory
manager immediate crashes Hostboot. It also fixes a heap corruption in
the SBE get capabilities command.

Improve debug ergonomics by allowing developers to backtrack from the
assertion through the call-stack. This is important for generic code,
for example, when dealing with assertions inside free().

Support SMF for P9N/P9C. Lots of minor tweaks to make this
work, but the biggest is to run userspace in problem state
This is needed because for SMF Hostboot will need to run in S=1,
HV=0,PR=1 (and kernel in S=1, HV=1, PR=0)
This commit makes P9 HB userpsace run in HV=0 PR=1 and kernel in
HV=1, PR=0.

This change implements a new global variable that indicates
whether the console daemon has been started. This variable
is checked in the doShutdown path prior to printing any
console traces.
There may be a situation very early in the IPL where we
attempt to write to console before the console daemon has
been started. This causes timeouts and IPL failures.

Current code has each Node calculate each Remote Node's IPC area
remote address by performing a fixed format calculation.
This change has each Node calculating its IPC area Remote address
and posting this value to a local SCOM register. A Node reads
a Remote Node's SCOM register to acquire the Remote IPC area
address.

The nanosleep takes seconds and nanoseconds as parameters but it
doesn't currently enforce that nsec doesn't exceed a sec.
The change here is to handle any overlaps to get the desired
results.

We hit out-of-memory errors while trying to boot a Opal system
due to the large amount of memory used by the PM procedures
and the pinning of memory in secure mode.
Also did some other rearranging of the pinned memory sections
to get some space back.

Some string utility functions are not being used in Bootloader, so
add conditions to keep them from being compiled in. Also memset
can be replaced by a simple loop to reduce size.

Move poll for empty queue out of kernel space to allow for
the primary thread to see new interrupts and clear out
incoming messages. This will prevent 2 nodes from deadlocking
one another while they wait for each other to handle the last
message that was sent to them.

See HW822317. The HMER register in P8 is not implemented to
handle multi-threaded XSCOM properly, so we need to move the
XSCOM mutex from per-thread to per-core. Also, there is an
issue where the 'done' bit can come on 1 cycle before the error
indicators, so need to potentially read the HMER a second time.
