* cpp.texi, extend.texi, gcc.texi, install.texi, invoke.texi,
tm.texi: Consistently refer to ISO C instead of ANSI C. Refer to
-std options alongside references to -ansi. Update some
documentation for C99.
* cpp.1: Regenerate.
