Based on a patch from Jesse Gross <jesse@nicira.com>
This switches the ixgb driver to use the new VLAN interfaces.
In doing this, it completes the work begun in
ae54496f9e8d40c89e5668205c181dccfa9ecda1 allowing the use of
hardware VLAN insertion without having a VLAN group configured.

skb_dma_map is incompatible with HW iommu due to the fact that multiple
mappings can result in different results each time. For this reason it is
best to just remove use of these function calls.

a) kernel developers suggest LLTX is broken and unsafe to use, remove it.
b) remember to pre-stop the queue if we won't have room
c) removing lltx means we can remove our tx lock

ixgb can remove irq_sem by auditing all the call sites to make sure
that each of them makes sure the adapter is in the correct state
before re-enabling interrupts. after doing this to all of our other
drivers it is becoming easier.

In order to remove the irq_sem code we need to implement strict
adapter state checking to prevent accidental double up or downs
or resets. This code is largely copied from e1000/e1000e.

Several devices have multiple independant RX queues per net
device, and some have a single interrupt doorbell for several
queues.
In either case, it's easier to support layouts like that if the
structure representing the poll is independant from the net
device itself.
The signature of the ->poll() call back goes from:
int foo_poll(struct net_device *dev, int *budget)
to
int foo_poll(struct napi_struct *napi, int budget)
The caller is returned the number of RX packets processed (or
the number of "NAPI credits" consumed if you want to get
abstract). The callee no longer messes around bumping
dev->quota, *budget, etc. because that is all handled in the
caller upon return.
The napi_struct is to be embedded in the device driver private data
structures.
Furthermore, it is the driver's responsibility to disable all NAPI
instances in it's ->stop() device close handler. Since the
napi_struct is privatized into the driver's private data structures,
only the driver knows how to get at all of the napi_struct instances
it may have per-device.

pci_enable_msi calls can fail for normal operational reasons. Driver
should not print an error message in that case. Fix a leak that leaves
msi enabled if pci_request_irq fails. We can remove CONFIG_PCI_MSI
ifdefs alltogether

We experimented with more descriptor buffer writebacks and found that
values larger than 8 give HW problems, but 8 is safe and gives us some
improved performance.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
