This is a new generic nmi_watchdog implementation using the perf
events infrastructure as suggested by Ingo.
The implementation is simple, just create an in-kernel perf
event and register an overflow handler to check for cpu lockups.
I created a generic implementation that lives in kernel/ and
the hardware specific part that for now lives in arch/x86.
This approach has a number of advantages:
- It simplifies the x86 PMU implementation in the long run,
in that it removes the hardcoded low-level PMU implementation
that was the NMI watchdog before.
- It allows new NMI watchdog features to be added in a central
place.
- It allows other architectures to enable the NMI watchdog,
as long as they have perf events (that provide NMIs)
implemented.
- It also allows for more graceful co-existence of existing
perf events apps and the NMI watchdog - before these changes
the relationship was exclusive. (The NMI watchdog will 'spend'
a perf event when enabled. In later iterations we might be
able to piggyback from an existing NMI event without having
to allocate a hardware event for the NMI watchdog - turning
this into a no-hardware-cost feature.)
As for compatibility, we'll keep the old NMI watchdog code as
well until the new one can 100% replace it on all CPUs, old and
new alike. That might take some time as the NMI watchdog has
been ported to many CPU models.
I have done light testing to make sure the framework works
correctly and it does.

In order to handle a new nmi_watchdog approach, I need to move
the notify_die() routine out of nmi_watchdog_tick() and into
default_do_nmi(). This lets me easily swap out the old
nmi_watchdog with the new one with just a config change.
The change probably makes sense from a high level perspective
because the nmi_watchdog shouldn't be handling notify_die
routines anyway. However, this move does change the semantics a
little bit. Instead of checking on every nmi interrupt if the
cpus are stuck, only check them on the nmi_watchdog interrupts.

We cannot assume that because hwc->idx == assign[i], we can avoid
reprogramming the counter in hw_perf_enable().
The event may have been scheduled out and another event may have been
programmed into this counter. Thus, we need a more robust way of
verifying if the counter still contains config/data related to an event.
This patch adds a generation number to each counter on each cpu. Using
this mechanism we can verify reliabilty whether the content of a counter
corresponds to an event.

Introducing *_text_reserved functions for checking the text
address range is partially reserved or not. This patch provides
checking routines for x86 smp alternatives and dynamic ftrace.
Since both functions modify fixed pieces of kernel text, they
should reserve and protect those from other dynamic text
modifier, like kprobes.
This will also be extended when introducing other subsystems
which modify fixed pieces of kernel text. Dynamic text modifiers
should avoid those.

Disable kprobe booster when CONFIG_PREEMPT=y at this time,
because it can't ensure that all kernel threads preempted on
kprobe's boosted slot run out from the slot even using
freeze_processes().
The booster on preemptive kernel will be resumed if
synchronize_tasks() or something like that is introduced.

In function kvm_arch_vcpu_init(), if the memory malloc for
vcpu->arch.mce_banks is fail, it does not free the memory

vcpu->arch.mce_banks is malloc in kvm_arch_vcpu_init(), but
never free in any place, this may cause memory leak. So this
patch fixed to free it in kvm_arch_vcpu_uninit().

kvm_handle_sie_intercept uses a jump table to get the intercept handler
for a SIE intercept. Static code analysis revealed a potential problem:
the intercept_funcs jump table was defined to contain (0x48 >> 2) entries,
but we only checked for code > 0x48 which would cause an off-by-one
array overflow if code == 0x48.
Use the compiler and ARRAY_SIZE to automatically set the limits.

Embedded PowerPC KVM has an exit timing implementation to track and evaluate
how much time was spent in which exit path.
For Book3S, we don't implement it. So let's not expose it as a config option
either.

When we queue an interrupt to the local apic, we set the IRR before the TMR.
The vcpu can pick up the IRR and inject the interrupt before setting the TMR,
and perhaps even EOI it, causing incorrect behaviour.
The race is really insignificant since it can only occur on the first
interrupt (usually following interrupts will not change TMR), but it's better
closed than open.
Fixed by reordering setting the TMR vs IRR.

This patch converts sparc (specifically sparc32) to use GENERIC_TIME via
the arch_getoffset() infrastructure, reducing the amount of arch
specific code we need to maintain.
The sparc architecture is one of the last 3 arches that need to be
converted.
This patch applies on top of Linus' current -git tree
I've taken my best swing at converting this, but I'm not 100% confident
I got it right. My cross-compiler is now out of date (gcc4.2) so I
wasn't able to check if it compiled. Any assistance from arch
maintainers or testers to get this merged would be great.

It doesn't account for phys_base like it should, fix by using
page_to_pfn().
While we're here, make virt_to_page() use pfn_to_page() as well, so we
consistently use the asm/memory-model.h abstractions instead of
open-coding memory model assumptions.

Make sure compiler won't do weird things with limits. E.g. fetching
them twice may return 2 different values after writable limits are
implemented.

Callers of copy_from_user() expect it to return the number of bytes
it could not copy. In no case it is supposed to return -EFAULT.
In case of a detected buffer overflow just return the requested
length. In addition one could think of a memset that would clear
the size of the target object.

OMAP GP timers keep running for a few cycles after they are stopped,
which can cause the timer to expire and generate an interrupt. The
pending interrupt will prevent e.g. OMAP from entering suspend, thus
we ack it manually. Only applicable on OMAP2/3/4.

Don't assume that gpmc_l3_clk is on, enable it before touching
configuration registers.
Note that the current code assumes that this clock is always
enabled. We are already setting smart idle and L3 autogating
for GPMC clock in gpmc_init.

We need to set the omap_chip.oc carefully for the clocks to work.
To fix this, set the omap_chip.oc in omap3_check_features() based
on the CONTROL_IDCODE and silicon revision registers.
Also add handling for 34xx es3.1.2 as es3.1 for now.
Fixes booting on at least overo board.

Make the omap_mux_read and write available for board code,
and rename omap_mux_set_board_signals into omap_mux_write_array.
Also add the related prototypes and comments into mux.h.
In some cases we want to change the signals dynamically,
mostly for power management.
Note that we cannot use the signal names as they are set
__init to save memory.

Looks like cmdline muxing got broken at some point when we
decided to limit muxing to __init code. Currently omap_mux_entry
list is not yet initialized when we try to initialize cmdline
muxing.
Fix this by calling omap_mux_init_list() before calling
omap_mux_set_cmdline_signals().

Currently, DSS does not wakeup when there is a DMA request. DSS wake
up event must be enabled so that the DMA request to refill the FIFO
will wake up the CORE domain.

During suspend, the kernel timekeeping subsystem is shut down. Before
suspend and upon resume, it uses a weak function
read_persistent_clock() to determine the amount of time that elapsed
during suspend.
This function was not implemented on OMAP, so from the timekeeping
subsystem perspective (and thus userspace as well) it appeared that no
time elapsed during suspend.
This patch uses the 32k sync timer as a the persistent clock.
NOTE: This does *NOT* fully handle wrapping of the 32k sync timer, so
more than one wrapping of the 32k sync timer during suspend may
cause problems. Also note there are not interrupts when the 32k
sync timer wraps, so something else has to be done.

Due to OMAP3 erratas 1.157, 1.185 the save of the last pad register
(ETK_D14 and ETK_D15) can fail sometimes when there is simultaneous
OCP access to the SCM register area. Fixed by writing the last
register to the save area.
Also, optimized the delay loop for the HW save to include an udelay(1),
which limits the number of unnecessary HW accesses to SCM register area
during the save.
