Having received another series of whitespace patches I decided to do this
once and for all rather than dealing with this kind of patches trickling
in forever.

Some architectures (alpha, mips, powerpc) have an arch-specific
"pci_probe_only" flag. Others use PCI_PROBE_ONLY in pci_flags for
the same purpose. This moves mips to the pci_flags approach so
generic code can use the same test across all architectures.

Declare pci_probe_only, etc. in asm-mips/pci.h file. This will fix
some sparse warnings.

The isa_slot_offset variable and its __ISA_IO_base macro is not used
anywhere anymore. It does not look like a decent interface per today's
standards either. Remove both including all places of initialization.

Correctly initialize io_map_base for the SB1250 PCI controller as required
for proper iomap support. Based on a proposal from Daniel Jacobowitz.

This is to break the code of people who think they are supposed to scribble
into the pci device structure - it's off limits.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
