Convert the incorrectly named PCIMEM_BASE to a variable called vga_base.
This removes the dependency on mach/hardware.h.

Convert PCIBIOS_MIN_IO and PCIBIOS_MIN_MEM to variables to allow
multi-platform builds. This also removes the requirement for a platform to
have a mach/hardware.h.
The default values for i/o and mem are 0x1000 and 0x01000000, respectively.
Per Arnd Bergmann, other values are likely to be incorrect, but this commit
does not try to address that issue.

Almost all implementations of pci_iomap() in the kernel, including the generic
lib/iomap.c one, copies the content of a struct resource into unsigned long's
which will break on 32 bits platforms with 64 bits resources.
This fixes all definitions of pci_iomap() to use resource_size_t. I also
"fixed" the 64bits arch for consistency.

nommu doesn't have any form of remapping support, so ioremap, etc
become stubs which just return the casted address, doing nothing
else.
Move ioport_map(), ioport_unmap(), pci_iomap(), pci_iounmap()
into a separate file which is always built.
