Mellanox ethernet driver update Oct-30-2014
The 1st patch from Saeed fixes a bug in the last net-next batch where
a VF could get access to set port configuration, the next patch from Amir
fixes a race in the port VPI logic. Next are two performance patches from Ido.
The patch to add checksum complete status on GRE and such packets was
preceded with a patch that converted the driver to only use napi_gro_receive
vs. the current code which goes through napi_gro_frags on it's usual track.
Eric D. has some thoughts and suggestions on that change for which we
want to take the time and consider, so for the time being dropped that
patch and the ones that depend on it.
Changes from V0:
- have the caller to provide the __GFP_COLD hint to the service function
- dropped the patch that changes the GRO logic and the subsequent dependent
patches.

Add code to issue CONFIG_DEV "get" firmware command.
This command is used in order to obtain certain parameters used for
supporting various RX checksumming options and vxlan UDP port.
The GET operation is allowed for VFs too.

Needed in order to get cache cold pages (L3 flushed) for HW scatter.
Otherwise memory may flush those entries when the packet comes from
PCI, causing back pressure resulting in BW decrease.

When IP_ALIGN has a non zero value, hardware will write to a non aligned
address. The only reader from this address is when copying the header
from the first frag into the linear buffer (further access to the IP
address will be from the linear buffer, in which the headers are
aligned). Since the penalty of non align access by the hardware is
greater than the software memcpy, changing the frag_align to always be 0.

We need to protect set_port_type() for concurrency, as the sysfs code could
call it from mutliple contexts in parallel.
The port_mutex is not enough because we need to protect from concurrent
modification of 'info' and stopping of the port sensing work.

Added wrapper to the ACCESS_REG command for handling guest HW
registers access, preventing write operations, but do allow reads.
This will prevent SRIOV guests to change port PTYS configuration,
such as speed/advertised link modes.

net_rx_action() can mask irqs a single time to transfert sd->poll_list
into a private list, for a very short duration.
Then, napi_complete() can avoid masking irqs again,
and net_rx_action() only needs to mask irq again in slow path.
This patch removes 2 couples of irq mask/unmask per typical NAPI run,
more if multiple napi were triggered.
Note this also allows to give control back to caller (do_softirq())
more often, so that other softirq handlers can be called a bit earlier,
or ksoftirqd can be wakeup earlier under pressure.
This was developed while testing an alternative to RX interrupt
mitigation to reduce latencies while keeping or improving GRO
aggregation on fast NIC.
Idea is to test napi->gro_list at the end of a napi->poll() and
reschedule one NAPI poll, but after servicing a full round of
softirqs (timers, TX, rcu, ...). This will be allowed only if softirq
is currently serviced by idle task or ksoftirqd, and resched not needed.

flow_limit in struct softnet_data is only read from local cpu
and can be moved to fill a hole, reducing softnet_data size by
64 bytes on x86_64
While we are at it, move output_queue, output_queue_tailp and
completion_queue, so that rx / tx paths touch a single cache line.

This reverts commit 68da166491655bc54051bf04c78ce648e2e33508.
It turns out that the assertion about scope of regressions due to
always keeping keyboard controller in legacy mode was proven wrong.
There are laptops, such as Clevo W650SH, that only have internal
touchpad (no external PS/2 ports), that require active multiplexing
mode to switch the touchpad (Elantech) into native mode instead of
basic PS/2 emulation.

In altera_ps2_close, the data register (offset 0) is written instead of
the control register (offset 4), leading to the RX interrupt not being
disabled. Fix this by calling writel() with the offset for the proper
register.

Expression haptic->pwm_dev->period * haptic->magnitude is of type
'unsigned int' and may overflow. We need to convert one of the operands
to u64 before multiplying, instead of casting result (potentially
overflown) to u64.

psmouse_reconnect() will not be called if psmouse driver is not bound to
the serio port, so there is no point in checking that. Also, as coded, it
introduces potential NULL dereference in psmouse_dbg() in case psmouse is
indeed NULL. Let's just remove it.

The bitmask comment says it will enable GPIO 8-14 and 16-20 for keypad use,
but it actually enables GPIO 8-11 and 13-20 due to a bit error.
Instead of masking of the "hole" at GPIO 12 (which is used for keypad
output 4) mask of the proper "hole" at GPIO 15.

Add the new flags argument to calls of (devm_)gpiod_get*().
Currently both forms (with or without the flags argument) are valid thanks
to transitional macros in <linux/gpio/consumer.h>. These macros will be
removed once all consumers are updated and the flags argument will become
compulsory.

"These are fixes received after my previous pull request plus one that
has been in the works for quite a while, but its previous version
caused problems to happen, so it's been deferred till now.
Fixed are two recent regressions (MFD enumeration and cpufreq-dt),
ACPI EC regression introduced in 3.17, system suspend error code path
regression introduced in 3.15, an older bug related to recovery from
failing resume from hibernation and a cpufreq-dt driver issue related
to operation performance points.
Specifics:
- Fix a crash on r8a7791/koelsch during resume from system suspend
caused by a recent cpufreq-dt commit (Geert Uytterhoeven).
- Fix an MFD enumeration problem introduced by a recent commit adding
ACPI support to the MFD subsystem that exposed a weakness in the
ACPI core causing ACPI enumeration to be applied to all devices
associated with one ACPI companion object, although it should be
used for one of them only (Mika Westerberg).
- Fix an ACPI EC regression introduced during the 3.17 cycle causing
some Samsung laptops to misbehave as a result of a workaround
targeted at some Acer machines. That includes a revert of a commit
that went too far and a quirk for the Acer machines in question.

- Fix a regression in the system suspend error code path introduced
during the 3.15 cycle that causes it to fail to take errors from
asychronous execution of "late" suspend callbacks into account
(Imre Deak).
- Fix a long-standing bug in the hibernation resume error code path
that fails to roll back everything correcty on "freeze" callback
errors and leaves some devices in a "suspended" state causing more
breakage to happen subsequently (Imre Deak).
- Make the cpufreq-dt driver disable operation performance points
that are not supported by the VR connected to the CPU voltage plane
with acceptable tolerance instead of constantly failing voltage

If a device's dev_pm_ops::freeze callback fails during the QUIESCE
phase, we don't rollback things correctly calling the thaw and complete
callbacks. This could leave some devices in a suspended state in case of
an error during resuming from hibernation.

If an asynchronous suspend_late or freeze_late callback fails
during the SUSPEND, FREEZE or QUIESCE phases, we don't propagate the
corresponding error correctly, in effect ignoring the error and
continuing the suspend-to-ram/hibernation. During suspend-to-ram this
could leave some devices without a valid saved context, leading to a
failure to reinitialize them during resume. During hibernation this
could leave some devices active interfeering with the creation /
restoration of the hibernation image. Also this could leave the
corresponding devices without a valid saved context and failure to
reinitialize them during resume.

If the regulator connected to the CPU voltage plane doesn't
support an OPP specified voltage with the acceptable tolerance
it's better to just disable the OPP instead of constantly
failing the voltage scaling later on.
Includes a fix to move initialization of opp_freq outside

Subject: ACPI / EC: Add support to disallow QR_EC to be issued when SCI_EVT isn't set
The behaviors of the 2 vendor firmwares are conflict:
1. Acer: OSPM shouldn't issue QR_EC unless SCI_EVT is set, firmware
automatically sets SCI_EVT as long as there is event queued up.
2. Samsung: OSPM should issue QR_EC whatever SCI_EVT is set, firmware
returns 0 when there is no event queued up.
This patch is a quick fix to distinguish the behaviors to make Acer
behavior only effective for Acer EC firmware so that the breakages on
Samsung EC firmware can be avoided.

Commit 6ab3430129e2 ("mfd: Add ACPI support") made the MFD subdevices
share the parent MFD ACPI companion if no _HID/_CID is specified for
the subdevice in mfd_cell description. However, since all the subdevices
share the ACPI companion, the match and modalias generation logic started
to use the ACPI companion as well resulting this:
# cat /sys/bus/platform/devices/HID-SENSOR-200041.6.auto/modalias
acpi:INT33D1:PNP0C50:
instead of the expected one
# cat /sys/bus/platform/devices/HID-SENSOR-200041.6.auto/modalias
platform:HID-SENSOR-200041
In other words the subdevice modalias is overwritten by the one taken from
ACPI companion. This causes udev not to load the driver anymore.
It is useful to be able to share the ACPI companion so that MFD subdevices
(and possibly other devices as well) can access the ACPI resources even if
they do not have ACPI representation in the namespace themselves.
An example where this is used is Minnowboard LPC driver that creates GPIO
as a subdevice among other things. Without the ACPI companion gpiolib is
not able to lookup the corresponding GPIO controller from ACPI GpioIo
resource.
To fix this, restrict the match and modalias logic to be limited to the
first (primary) physical device associated with the given ACPI comapnion.
The secondary devices will still be able to access the ACPI companion,
but they will be matched in a different way.

For boards without a reset GPIO we skip the delay between enabling the
pcie_ref_clk and touching the RC registers for configuration. This hangs
the system if there isn't a proper delay to ensure the clocks are settled
in the DW PCIe core.
Also iMX6Q always needs an additional 10us delay to make sure the reset is
propagated through the core, as we don't have an explicitly controlled
reset input on this SoC.
This fixes a problem with 3fce0e882f61 ("PCI: imx6: Delay enabling
reference clock for SS until it stabilizes"): the kernel doesn't boot on
systems that don't pass the PCI GPIO reset in the DTB. This regression
affects mx6 nitrogen boards.

Rusty noticed a Really Bad Bug (tm) in my NT fix. The entry code
reads out of bounds, causing the NT fix to be unreliable. But, and
this is much, much worse, if your stack is somehow just below the
top of the direct map (or a hole), you read out of bounds and crash.

ext4_ext_convert_to_initialized() can return more blocks than are
actually allocated from map->m_lblk in case where initial part of the
on-disk extent is zeroed out. Luckily this doesn't have serious
consequences because the caller currently uses the return value
only to unmap metadata buffers. Anyway this is a data
corruption/exposure problem waiting to happen so fix it.

When clearing inode journal flag, we call jbd2_journal_flush() to force
all the journalled data to their final locations. Currently we ignore
when this fails and continue clearing inode journal flag. This isn't a
big problem because when jbd2_journal_flush() fails, journal is likely
aborted anyway. But it can still lead to somewhat confusing results so
rather bail out early.

When ext4_handle_dirty_dx_node() or ext4_handle_dirty_dirent_node()
fail, there's really something wrong with the fs and there's no point in
continuing further. Just return error from make_indexed_dir() in that
case. Also initialize frames array so that if we return early due to
error, dx_release() doesn't try to dereference uninitialized memory
(which could happen also due to error in do_split()).

ext4 does not permit changing the metadata or journal checksum feature
flag while mounted. Until we decide to support that, don't allow a
remount to change the journal_csum flag (right now we silently fail to
change anything).

When we fail to load block bitmap in __ext4_new_inode() we will
dereference NULL pointer in ext4_journal_get_write_access(). So check
for error from ext4_read_block_bitmap().

When there are no meta block groups update_backups() will compute the
backup block in 32-bit arithmetics thus possibly overflowing the block
number and corrupting the filesystem. OTOH filesystems without meta
block groups larger than 16 TB should be rare. Fix the problem by doing
the counting in 64-bit arithmetics.
