[SCSI] bfa: IOC auto recovery fix.
- Made IOC auto_recovery synchronized and not timer based.
- Only one PCI function will attempt to recover and reinitialize
the ASIC on a failure, after all the active PCI fns
acknowledge the IOC failure.
